Dr. Sheba Naqvi is a designated Civil Surgeon by USCIS to conduct immigration medical examination for adjustment of status. The USCIS Form I-693 is $380 for adults, and $300 for children 12 and under. You will need to provide vaccination records, passport, and ID at time of appointment. Completion of the USCIS immigration forms will take 7-10 business days for pickup. At appointment, we will complete blood work to determine which vaccinations may be missing.
